In the self-disclosure form you will provide information about your product and company which will then be published as part of the product entry on the Cologne List. In addition you will answer two questions which will allow athletes to assess the risk of your product being contaminated with anabolic-androgenic steroids.

Furthermore, under the section "Manufacturers' Comments" the company has the option to publish further information about the product in question. This information must be of a primarily informative nature; for example, more detailed descriptions about the product contents or its certification from institutes.

The section „Product Category“ is a drop down menu in the relevant PDF-form. When you click on the right end of the field, the available categories will be displayed. If none of the categories are applicable to your product, please choose “other products”.
